The Reablement team assists patients requiring additional support and rehabilitation on discharge from hospital, and those in the community who need increased care or therapy input. Assessments are holistic and focused on maximising independence at every stage. Patients are at the heart of everything we do.
This Post is Based in Wychavon and is offered on a full time basis, with flexible working and alternative hours considered.
Main duties of the job
As a member of the reablement team, you will work closely with therapists and social care teams, having the unique opportunity to experience both sectors whilst with us.
You will perform holistic, goal focused, patient centred assessments, and develop thorough treatment programmes to meet the goals. You will manage a varied caseload, spanning all Occupational Therapy specialities.
You will work closely with both health and social care colleagues, in a blended approach to patient care. Physiotherapists, nurses, occupational therapists, therapy assistants and social care staff undertake collaborative working on a daily basis to ensure that all patient needs are met. Regular clinical supervision and professional support are provided. We positively encourage any special interests which complement the patient demographic that we see
You will be supported in developing leadership skills and widening your Clinical knowledge to be able to reach Band 6 Level through an individualised plan.
