Work across all clinically appropriate patients, 17-years-old and over, to deliver evidence-based interventions.
Under the direction of Service Lead and as part of the Community Mental Health Service, provide prompt local assessment, triage, risk assessment, risk management, signposting, onward referral and evidence-based interventions for people presenting with mental health problems.
Represent the team on specific projects and working groups as required.
Main duties of the job
Will work as a key member and resource of the MDT core team.
This post will offer a challenging and exciting opportunity to gain experience of working across North Community Mental Health Service, developing relationships with Primary Care, partner agencies and wider mental health services.
You will have the opportunity to design and deliver evidence-based recovery focused mental health interventions and groups. Knowledge of delivery psychoeducation within a group environment and 1:1 would be an advantage.
You will have experience of working with this user group in community settings. You will have a registered qualification in mental health nursing, occupational therapy or social work.
You will hold a blended caseload of common mental health disorders, severe mental illness patients, Primary and Secondary, including those with emotional needs.
The ability to work flexibly as the need of the service demands is essential.
Attend weekly MDT meetings, participate in professionals meetings, signpost to adjunct services depending on patient need.
Support and guidance to the carers of patients accessing the services
