Community Dementia Nurse - Hereford Memory Services

Job summary

Community Dementia team 22.5 hours per week.

We have an exciting opportunity for a Qualified Nurse to join our well-established Community Dementia Team which is a countywide service. The team provides low-level support to individuals with a dementia diagnosis and dementia assessments.

The service is open Monday to Friday 9-5 excluding bank holidays. Flexible working patterns will be considered.

The team is based in the grounds of the Belmont Abbey, which is peaceful rural location, 2 miles from the main city of Hereford. There is plenty of free parking.

Main duties of the job

Manage a caseload of patients to ensure they receive a timely assessment, diagnosis and treatment.

Monitor treatments.

Effective and timely discharge planning and coordination.

Undertake and understand physical health observations and take appropriate action where necessary.

Monitor, evaluate and modify treatment in order to measure progress and ensure effectiveness of intervention.

Apply a high level of understanding of the effect of dementia and provide advice on management of the condition, healthy lifestyles and well-being.

Carry out complex risk assessment.

Timely and accurate reporting and escalation of incidents and safeguarding concerns.

Attend regular multidisciplinary meetings.

Participate in undertaking professional cover/duty.
