An exciting
opportunity has arisen for an enthusiastic individual to join our Housing,
Adaptations and Specialist equipment (HASE) occupational therapy team. We provide
comprehensive OT assessments relating to housing, major adaptations (DFGs),
minor adaptations, and specialist equipment for residents of Worcestershire.
This is a permanent
post, for 29.75 hours a week, over three plus days with flexibility.
We offer a
comprehensive induction programme relating to major adaptations and DFGs. We
offer and support continuing professional development to equip you with the
skills and knowledge required for this post. We provide regular peer and 1:1
supervision.
Housing,
equipment, and adaptations for all age groups is our specialism and the core

Main duties of the job
You will
carry out functional assessments within the home and, working with the resident
and their carers, identify the challenges faced in remaining at home safely
whilst managing everyday tasks.
Having
identified the challenges, you will use robust clinical reasoning to make
recommendations in relation to the provision of major adaptations, housing and
rehousing, specialist and none specialist equipment, and minor adaptations.
You will
work in partnership with other health professionals, Local Councils, and the
Home Improvement Agency to help deliver these recommendations with the aim of
promoting independence, safety, quality of life and good health outcomes for
residents in Worcestershire.
You will be
responsible for the recording, planning, and managing your own caseload.
Whilst
based within a supportive team, you will have autonomy to balance and achieve
the demands of this busy, varied but rewarding role.
You will be
an excellent communicator, working with clients, carers and other professionals
to achieve excellent outcomes for your clients. You will need to be
self-motivated, have an excellent work ethic, be able to work autonomously,
whilst being a team player.
You will
participate in the process of triaging referrals to the HASE OT Service to
ensure clients needs are being met by the most appropriate service and person.
You will contribute to the smooth running of an
office, team and to wider service development.
