to remain in their own home as the first alternative to inpatient admission.
The Neighbourhood Team consists of
Physiotherapists, Occupational Therapists, Community Nurses, Senior Nurse Practitioner, Healthcare
Assistants and administrators. GP and social care colleagues work closely
alongside.
The
Neighbourhood Team operates from 7:30am to 18:30pm on a seven-day rota
including weekends and Bank Holidays. You will be supported to work flexibly
within these hours.

Main duties of the job
Your
typical working day will include triage and allocation of planned and unplanned
visit requests for physiotherapy interventions. This may involve urgent
response assessment as part of the teams admission avoidance remit, supporting
complex hospital discharges and planning domiciliary interventions to facilitate
independent living. You will work alongside physiotherapy, nursing and OT
colleagues in the same team. We have close links with Worcester and Birmingham
universities in support of student placements.

Working as an autonomous practitioner within
an integrated team you will provide timely and expert professional assessment
and treatment interventions to patients either in their own homes or other
community settings. This will be achieved through a combination of hands on
expert clinical skills and knowledge as a Physiotherapist to treat, educate and
support patients and by working with patients and their families to assess, plan
and implement appropriate care plans and treatment.
Provide
case management for a designated caseload of patients who have immediate and /
or complex care needs.
Triage patients to ensure prioritisation of
clinical need.
Develop and co-ordinate individualised
rehabilitation plans in close liaison with the team, the patient and their
carers as well as all other services.
Work holistically with all team members,
carrying out joint visits and actively participating in the integration of services - increasing the knowledge of all team members through case discussion and joint working.
