An exciting opportunity has arisen for a Trainee qualified ACP to
join the Hereford City Community Mental
health Neighbourhood Team based at Widemarsh Street. We are a forward thinking

The ACP trainee will contribute to the support and development of the
team, and through development use the four pillars to influence and underpin
service delivery and design. You will
be supported within the team by both the clinical and leadership structure including
the ACPs during the training. You will be shaping our services and ensuring
that ultimately our patients and service users get expert care focused on
recovery and independence from a non-medical prescribing provision, whilst
working as an autonomous advanced practitioner who will complement the service
provision.
Full time post 37.5 hours per week, over 5 days.
Main duties of the job
The post holder will work with a degree of professional
autonomy appropriate to level of training and begin to make critical/complex
decisions with the support of qualified advanced clinical practitioners and the
medical team.
Undertake advanced clinical assessment appropriate
to level of training of patients who have complex and or chronic disease
presentations, including screening patients for disease risk factors and early
signs of illness, making a differential diagnosis, prescribing treatments as an
independent non-medical prescriber, initiating and reviewing treatment plans
and discharging patients.
Working with a range of agencies internally and externally,
signposting to appropriate sources of support providing short to long-term
interventions to support recovery. Provide guidance and support to the carers
of patients accessing the services and our partners.
To be able to travel throughout the county as
required. To supervise junior staff and students. The
ability to be flexible and organised is essential.
