Job summary

We are seeking an experienced Senior Clinical Practitioner (or ACP please see further advert), who has a passion for frailty, to join our Care Home team based in Malvern.The Care Home Team undertakes the reactive and proactive care for patients in Residential Care and Nursing Homes within the locality working in partnership with the PCN colleagues and alongside dedicated GP support. The successful candidate will join the established team in working together to ensure the delivery of high quality care for people within local Care homes.

As an experienced practitioner you will have completed the Advanced Assessment Skills and Non-Medical Prescribing (V300) modules

Main duties of the job

The ability to work independently is essential as the post holders will need to offer urgent care, treat minor illness, give patient advice and offer chronic disease management. You will have the ability to prioritise and triage the needs of patients, making any referrals for further investigations where appropriate in line with local and practice guidelines, whilst ensuring your clinical practice is safe and effective; you will be fully supported within a collaborative, multidisciplinary team.
