This is a highly

Mortimer ward is a
21 bedded acute Mental Health Ward situated in The Stonebow Unit, Hereford and
is part of the provision of acute inpatient services across Herefordshire and
Worcestershire. It is a vital service for supporting those with acute mental
health difficulties of varying complexities. Patients requiring admission may
also have complex physical co morbidities and applicants should have an
interest in both mental and physical health care delivery. Patients admitted
can both be informal or detained under the Mental Health Act and require a
holistic care approach to work towards their Recovery.
The ward has a
multi-disciplinary team approach including nurses, physiotherapists,
occupational therapist, activity support workers, Psychiatric Consultants and
junior doctors. You will also work alongside colleagues in the Crisis Team and
Adult Social Care. We also support student nurses as part of our team.
Rotational shift
pattern system over a 24 hours / seven day
period.(07.30hrs-20.00hrs & 19.30hrs-08.00
hrs).Full time but part time hours considered. The Stonebow Unit is
currently undergoing extensive redesign which will enhance the patient
experience, ward safety and therapeutic environment.
This role will
maintain a strong clinical focus on the ward to support the maintenance of
quality and leadership for the patient experience.
Main duties of the job
- Maintain high standards of patient
care ensuring all patients individuality, dignity and privacy is maintained at
all times.
- Provide key leadership in MDTs
reviews, communicating treatment plans to the wider team & liaising with
external agencies
- Ensure that each patient receives an
individual clinical assessment which is the subject of a regularly updated care
plan that, wherever practicable, is formulated in conjunction with the patient.
- Ensure that information regarding patients
needs and progress are communicated promptly & accurately to their
relatives & carers having due regard for confidentiality.
- Provide strong clinical leadership
for all staff, maintaining & enhancing own knowledge by making use of
educational opportunities, under the guidance of the Ward Manager. This
includes appropriate extensions to the role of the nurse/OT in line with
NMC/HCPC & Trust policy.
- Maintain & promote the importance
of physical health needs amongst our service users with serious mental illness
- Develop additional clinical skills
such as ECG & venepuncture
- Taking a role in the monitoring &
auditing of medication
- Taking a lead in safeguarding
- Taking a lead with inducting new
staff
- Take a lead in ensuring that students
in practice are supported & receive a positive experience
- Shaping clinical knowledge and skills
with other staff
- Ensuring that all staff members
receive regular Clinical supervision
- Interest in further educational
courses to enhance clinical development
