evidenced based dementia assessment and post diagnostic interventions to people
presenting with early symptoms of dementia. We are continuously looking to
develop and enhance the service we offer, and this includes many opportunities
to develop as individuals through both formal an informal training and
supervision.
We are looking to recruit a flexible, motivated and
enthusiastic person with excellent communication skills to develop this new
post within our supportive and valued team. It
will be an exciting opportunity to be part of establishing and embedding the
role of the band 4 Nursing Associate withing the service. We have lots of ideas
about what this role might look like, but want to wait for the right candidate
to be appointed so that they can take an active role in their posts development.
The
teams base is in Kidderminster Hospital, but we offer a county wide service
with the flexibility of working from home or other trust bases around the
county.

Main duties of the job
We
are looking for a Nursing Associate with a special interest and experience in
dementia. You will work alongside the multi-disciplinary team to support patients
and their families through a dementia assessment and post diagnostic
interventions. This will include supporting with dementia assessments,
completing medication reviews, signposting and referring patients onto other
services, delivering groups and supporting with psychological adjustment to a
diagnosis.
You will be given the opportunity to build your
confidence and skill set withing the team whilst working independently under
the leadership and supervision of registered nurses.
There
will be opportunities to supervise, assess and mentor Support Workers and
assist in the supervision and learning of other health care students.
The post is for 22.5 hours which can be worked
over a minimum of 3 days. Specific days/ hours can be negotiated.
