Senior Occupational Therapist Enhanced Dementia Care Service

Job summary

The Enhanced Dementia Care Service is a true example of integrated working, being delivered in partnership with Hestia and Hounslow Health Primary Care Network. The service works with people living with dementia in Hounslow to enable them to live well at home, plan for changes in their health or wellbeing and prevent crisis.

Successful candidates for this post will need to be self-motivated, forward thinking and committed to learning. You will proactively build professional relationships and work in collaboration with the multi-disciplinary team (MDT), patients living with dementia, their families and carers along with other members of the local health / support services.

You must be flexible and responsive with excellent time management skills as the team is required to be reactive and priorities change frequently throughout the day.

Main duties of the job

We are looking for a confident and autonomous Occupational Therapist with relevant experience to work as the sole Occupational Therapist within a multidisciplinary team.

You will be required to provide holistic clinical assessments as well asspecialist assessments of function and occupational performance to inform care planning.

You will bring a comprehensive understanding of the role of Occupational Therapy in a community setting and within the context of Dementia. We are looking for someone who is enthusiastic and able to draw on their experience to deliver high quality clinical services to the population, whilst offering specialist input within the team. You will also bring an understanding of how to support individuals and their carers to plan for and prevent crisis in order to maintain quality of life and wellbeing at home.

  • Broad range of core clinical specialties with ability to relate to the advertised role

Desirable

  • Involvement in audit, research and understanding quality improvement process in the context of relevant national guidelines
  • Experience of supervising therapy assistants, junior staff or students
