Job summary

We have an exciting opportunity for a Specialist Diabetes Dietitian to join the multidisciplinary Hounslow Community Diabetes service. This service is commissioned to provide a specialist integrated diabetes services to adults with Type 2 diabetes in the London Borough of Hounslow, with the specialist dietetics team delivering X-PERT structured diabetes education and specialists diabetes clinics. The model of working is hybrid - a mixture of remote and face to face activity and can include evening or weekend shifts, depending upon service needs.

The candidate should be able to demonstrate advanced knowledge and experience of diabetes clinical management . We are also looking for a Dietitian who has experience of delivering Diabetes Structured Group Education sessions e.g X-PERT diabetes ; X-PERT insulin or a similar model of educational delivery.

The team works closely with the Hounslow Primary Care Networks (PCNs), including PCN Dietitians and the Diabetes Multi-Disciplinary Team (MDT)

We are looking for candidates who are enthusiastic and adaptable to change, with excellent clinical skills in Type 2 diabetes management and competence in use of IT systems.

This role will require travelling to community venues, you must therefore have access to your own transport and the ability to transport educational resources, as required.

Main duties of the job

  • The role supports the weekly consultant-led multidisciplinary team meetings and supports joint working and caseload management primarily with the diabetes specialist nursing team.
  • To provide specialist diabetes dietetic assessment, treatment and structured education programmes to clients with type 2 Diabetes within the borough of Hounslow. The role is delivered in a variety of settings including education venues, GP practices, clinics and client homes and employs both virtual and face to face modalities.
  • To be responsible for triaging referrals and organising own clinical caseload and administrative tasks, and to be flexible to the needs of the service as indicated by the clinical lead/line manager.
  • To act as a source of expertise to key partners and other Health Professionals.
  • To ensure patients have timely access to specialist dietetic expertise within the specified key performance indicators.
  • To initiate and participate in clinical governance activities across HRCH Dietetics, and to collect and record output data to indicate service improvement.
  • To ensure primary health care teams and non-NHS organisations have access to up to date nutritional information through the development of education and training materials and sessions.
