Job summary

We are looking for enthusiastic PCN Pharmacy Technicians (Band 5) to complement our existing Pharmacy team and work closely with Practice teams in Chiswick, Feltham&Bedfont, Great West Road or Hounslow Health PCNs in Hounslow Borough.

The PCN Pharmacy Technicians will be instrumental in supporting the development and implementation of quality prescribing initiatives, new Repeat Prescribing Service and, in the process help standardise care processes and reduce inequalities across the PCNs. They will provide support to PCN Pharmacists for medicines-related issues in Practices and Care Homes, carry out quality improvement projects and be part of a dynamic team to improve care and services.

Main duties of the job

You will work as part of a multidisciplinary team to:

  • Support the development and the running of processes for repeat prescription re-authorisation
  • Work with practices to enable them to develop and apply systems for safer prescribing
  • Develop good relationships between local community pharmacies, and the PCN pharmacy team
  • Support pharmacist colleagues with the delivery of structured medication reviews
  • Support PCN Pharmacists with the management of medicines on transfer of care: discharge letters, treatment advice notes and medicines reconciliation

  • Up-to-date clinical knowledge who understand the nature of primary care prescribing, medication review, and strategies for improving prescribingA commitment to addressing health inequalities and proactively reaching people from all communities
  • The ability to recognise their personal limitations and boundaries, to understand those of their colleagues, and refer to other colleagues when necessary
  • The ability to thrive when working in a devolved, flexible, autonomous environment
  • Have the personality, skills and the attitude and aptitude to work within a fast paced and ever evolving primary care environment
