Job summary
Great West Road Primary Care Network is a well-established group of eight GP Practices, committed to providing excellent patient centered care for 70,000 patients in the Great West Road area.
We are looking for an advance clinical practitioner (ACP) pharmacist to lead on our cardiovascular disease programmes. You will work closely together with multi-professionals from both the primary care network and general practices with the focus on delivering core services in cardiovascular disease to our local population. In this role you will use your skills in diagnostics and therapeutics, enhanced level consultation, critical thinking with clinical decision-making, and leadership.
Candidate should ideally already be practicing as an ACP. You would have a portfolio which demonstrates the four pillars of advanced clinical practice.
Main duties of the job
We are most interested in candidates with:
- Up-to-date clinical knowledge who understand the nature of primary care prescribing, medication review, and strategies for improving prescribing
- Scope of practice in acute illness and repeat prescribing is desirable
- Scope of practice evidenced in cardiovascular and diabetes management
- The ability to thrive when working in a devolved, flexible, autonomous environment
- You will be practicing at a senior level, have excellent written and verbal communication skills, experience of influencing others, conducting and managing face to face and remote clinical caseloads in primary care
- Have the personality, skills, attitude and aptitude to work within a fast paced and ever evolving primary care environment

- Master's degree in Pharmacy or equivalent
- Post-graduate clinical diploma level or equivalent training/experience or Independent prescriber
- Registered as a Pharmacist with the General Pharmaceutical Council
Desirable
- Management/leadership qualification such as Diploma in Management, Master in Business Administration, NHS or CPPE Leadership Academy
- A member of or working towards Faculty membership of the Royal Pharmaceutical Society
Experience

- Experience of leading and managing staff
- Experience of delivering research projects and influencing changes in practice.

- Work autonomously
