Job summary

This is a key role in the provision of an integrated specialist nursing service for residents requiring IV therapy who are registered with a GP in the London Borough of Richmond.

The post holder will coordinate the delivery of IV therapy by the Adult Community Nursing Services and where possible it will be expected that the post holder will undertake the initial visits either alone or in conjunction with a member of one of the Community Nursing Teams.

Main duties of the job

In addition, the post holder will be expected to provide support and advice which includes:

  • Supporting the development of the service.
  • Coordinating competency-based training
  • Facilitating the provision of high-quality care for these patients within the community ensuring evidence-based practice and innovative models of care are always delivered.
  • Evaluating clinical pathways, policies, and procedures for managing IV therapy across primary, community, and secondary care.
  • Providing local leadership, consultancy. and advice to other professionals, patients, and their carers
  • Developing close working partnerships with the acute partners; other secondary care providers local, other health care professionals and voluntary services involved in the patient's care.

The Clinical Services Directorates have a wide range of responsibilities to manage, including coordinating and delivering community primary care services on behalf of the Trust. A wide range of these healthcare services are delivered by staff in the community, who include medical staff, paediatric therapists, therapy assistants, health visitors, specialist nurses, nurses, nursery nurses, paediatric phlebotomists and healthcare support workers with the support of administrative staff working in localities with GPs, other health professionals, early years, education, and social care professionals.

The digitalisation of health services is underway with a requirement to become paper-light by 2023 through improving the use of digital tools and adopting mobile and agile working practices and increasing the efficiency and effectiveness of clinical delivery.

  • Qualified RGN (Registered General Nurse - Adult) with current registration with NMC - Part1
  • Relevant post registration study at diploma/degree level and relevant CPD e.g., IV, palliative care, tissue viability
  • Assessor/Supervisory/ Mentorship Preceptorship or equivalent (willing to undertake)
  • Experience working in the acute or in the community setting for more than a year as a registered nurse.

Desirable

  • Clinical Assessment Skills
  • Nurse Prescriber V300 (willing to undertake)
  • Diploma or BSc (Hons) Community Health (DN)

Knowledge

  • Understanding of the impact of Mental Health e.g., dementia, depression, on the management of medical and long-term conditions
