Job summary
We have an exciting 12 months secondment opportunity to join the Cardiac Rehab team to develop your skills in cardiac rehabilitation. We are looking for a dynamic team member to support the team to deliver clinics and classes across Hounslow and Richmond.
This will involve full time commitment, under the supervision of the Cardiac-rehab and specialist nurse service lead. You will receive appropriate training and supervision with our physiotherapists.

Main duties of the job
The successful candidate will work as part of the Cardiac Rehabilitation Multidisciplinary Team, providing a specialist rehabilitation service to patients with a range of cardiac conditions across the organisation within outpatient clinics, Cardiac Rehabilitation exercise classes and Heart Health education sessions. They will support and advise patients and their families during their pathway through the service.
JOB PURPOSE:
- To manage own caseload, assess plan and implement and evaluate interventions using evidence based/best practice, working towards patient centred goals in the context of the specialty and as part of the multi-disciplinary team.
- To be accountable for their own individual decisions and professional actions
- To participate and flexibly assist senior colleagues in the wider service needs and development
- Comply with the Chartered Society of Physiotherapy Care Standards of Practice and Health Professionals Council Standards of Performance Conduct and Ethics of all other relevant national and local policies and procedures
- To assist the Lead Physiotherapist in the provision of the Physiotherapy Service as necessary
