Job summary

Hounslow and Richmond Community Health are looking for two Advanced Practitioners, these post have been developed for the purpose of leading and embedding excellent care standards and pathways for people in our Communities on a Frailty and/or End of Life pathway (EoLC).

The roles are required to:

  • Co-ordinate and contribute to the development and delivery of the trusts Frailty and End of Life Strategies.
  • Lead on related policies, providing assurance that Frailty and End of Life care is safe, effective, caring, responsive and well led within the Trust.
  • Provide trust wide clinical leadership for the development, implementation, and evaluation of our services.
  • Liaise and build relationships with relevant colleagues and clinical experts in other organisations to establish excellent seamless pathways for patients.
  • Support clinicians across the multidisciplinary services providing frailty, palliative and end of life care to people in the community to ensure a seamless delivery of high quality care that anticipates the needs of the population and helps people to live well in their last few years.
  • Provide specialist advice to patients and/or their carers to influence better care and outcomes for patients.

Main duties of the job

  • Co-ordinate and contribute to the development and delivery of the trusts Frailty and End of Life Strategies and related policies, providing assurance that our service is safe, effective, caring, responsive and well led within the Trust.
  • Work with partner agencies in the design and review of services that optimise care in the right place at the right time for this patient population.
  • Provide trust wide clinical leadership for the development, implementation, and evaluation of frailty and end of life care in clinical services.
  • Liaise and build relationships with relevant colleagues and clinical experts in other organisations to establish excellent seamless pathways.
  • Act as an expert clinical advisor providing leadership and highly specialist professional advice, guidance and support to community teams, to ensure that safe, effective and high quality evidence based end of life care is delivered .
  • Work closely with health care practitioners, coordinating more complex frailty and end of life care, ensure our staff across the trust, are confident and clinically competent to recognise dying, to make informed decisions on care needs and deliver safe and effective care.
