Job summary

Hounslow Looked After (LAC) Children's Service are seeking a dynamic and innovative Health Visitor / School Nurse to join the team.

This is a demanding specialist role requiring high levels of professional skills and practice and the ability to work as part of a team. You should have specialist knowledge of the LAC agenda.

You will have excellent Safeguarding and communication skills which can be demonstrated at interview. You will have experience of working as part of a multi-agency group.

Main duties of the job

You will play a key role in helping to improve the life chances of Looked After Children and promote outcomes for this vulnerable group. You will be expected to lead and co-ordinate on health issues, contribute and evaluate individual care programmes, develop and maintain collaborative working relationships and establish both effective systems and health promotion programmes for young people and their carers.

To promote and deliver a dedicated health service to children and young people who are looked after, working in conjunction with the designated doctors, relevant health services, foster carers, social services and the education department to ensure statutory health requirements and targets are achieved and health outcomes improved.
