Job summary

An exciting opportunity has arisen for a committed, enthusiastic individual to join our forward-thinking Community Teams in a new post as a Trusted Assessor for our Integrated Discharge Team.

You would be joining our Community Division at an exciting time as we progress on our journey to outstanding, having achieved a CQC rating of good overall in 2019 including Outstanding for Care in 2021. Our transformational achievements include the extensive use of technology enabled care and the development of integrated multi-disciplinary and multi-agency teams, in conjunction with a wide range of partners across the Island and mainland. The team has a wide breath of knowledge including Occupational Therapy, Physiotherapy, Social Work and Nursing.

A recent organisational change has established a new collective leadership and management structure across our Community and Mental Health and Learning Disabilities Divisions. This change is transformational for both divisions, aiming to establish a more resilient service management structure to better support our new models of care in terms of multi-agency and multi-disciplinary working, as well as creating capacity for development of the partnership between both divisions and our wider partners across the local system.

The post holder will be passionate about hospital flow and the 'home first approach'.

Main duties of the job

As Trusted Assessor for the Integrated Discharge Team (IDT), you will report directly to the Integrated Trusted Assessor Team Lead and be working to assess patients in the Acute hospital to support flow from the Acute beds to patient's homes with appropriate care or to community bedded settings.

The division is keen to hear from individuals with:

  • A keen interest and extensive experience working with adults in the Acute hospital setting who have the skills to ensure that patients have the appropriate care and support post discharge.
  • Excellent communication skills and the ability to work well as part of a team
  • An ability to effectively manage and prioritise conflicting workloads and deadline

Job Purpose

The accountability within your role is in line with the Nursing and Midwifery council (NMC) and / or the Health and Care professionals council (HCPC), which says that you must work together for the safeguarding and wellbeing of the public

Act as a Trusted Assessor for the Integrated Discharge Team (IDT) within the Acute hospital for packages of care, community beds across health and social care and third sector providers

Provide supervision, education, and training to the SPA and Community teams

Act as an advocate for patients to ensure the highest levels of care are delivered.

Safeguard the delivery of high standards of care

Deputise for the SPA Senior Clinicians to cover the oversight of the SPA and Trusted Assessment to community beds in their absence as and when required.

In conjunction with the Community Rehab senior clinicians provide effective clinical leadership.

Responsible for the delivery of high standards of nursing / therapy care to all patients.

Responsible for the clinical standards of care delivered within the service area.

Job Statement
