Job summary

We are looking for an enthusiastic full-time Occupational Therapist to join our Community Enhancing Recovery Team (CERT). CERT provide intensive support in people's homes as an alternative to being admitted to hospital. OT is a core discipline within the multidisciplinary team at CERT. This is an exciting opportunity for those wanting to work in community based, rehabilitation focused work.

You will receive regular supervision, have CPD opportunities, join the band 5 support group/network and be able to contribute to the development of the service.

Main duties of the job

  • To manage a defined caseload, using evidence based/client centred principles to assess, plan, implement and evaluate interventions in community settings and on home visits.
  • To undertake occupational assessments, formulations, care planning, address occupational performance and skills acquisition.
  • To maintain clinical records
  • To working collaboratively with MDT colleagues, and wider services, on care plans, service delivery and evaluation.
  • To develop skills and knowledge through participation in preceptorship and PDR
  • To undertake any other duties, which may be allocated by the Occupational Therapy Clinical Lead within the general scope of the post and appropriate to the grade.

To undertake occupational therapy assessments for a designated caseload, addressing occupational performance and skill deficits, enabling the client in areas of self-maintenance, productivity and leisure.

To work with clients to identify OT goals as part of the overall care plan

To monitor and evaluate treatment in order to measure progress and ensure effectiveness of the intervention

To establish effective communication networks with users and carers, team members, OT colleagues and other agencies

To provide support and education to clients and carers regarding aspects of occupational therapy e.g. equipment fitting, rehab programmes
