Job summary

The Urgent and Crisis Service is a multi-disciplinary team which operates 24 hours a day 7 days a week 365 days a year. It provides a service to people who are over 18, who are registered with a GP practice in the City of Sheffield.

The Urgent and Crisis Service accepts referrals from a wide range of sources, including self-referrals, referrals from service user family members, referrals from the police and ambulance services, social services, AMHPs and other health care professionals (e.g. GPs).

The service provides evidence-based assessments in line with NICE guidelines for Mental Health and the UK triage rating scale and interventions for those in crisis for up to 72 hours. The team will work closely with the NHS 111 mental health provision, Home Treatment Team, Approved Mental Health Practitioners, and the Decisions Unit as part of the offer of care.

To provide urgent access to a mental health assessment to service users presenting in a mental health crisis. Establishing the urgency of service response is undertaken utilising the UK triage rating scale and communicating the outcome of the triage with the SBAR format (Situation, Background, Assessment, Recommendation).

Main duties of the job

  • Attendance/medical leadership at regular team huddles/multidisciplinary team meetings,

  • Face-to-face crisis assessments of new service users, both at the Longley Center and if required, at their homes,

  • Provision of medical reviews for patients as part of their 72-hour intervention if indicated, e.g., to review current psychotropic medications and PRN medications,

  • Medical/psychiatric assessments, formulations, and risk assessments, particularly for complex presentations, as part of 72-hour intervention.

  • Assessments under the Mental Health Act,

  • Completion of administrative tasks necessary for clinical responsibilities (e.g. writing to GPs and updating referrers),

  • Liaison with members of other teams (e.g. home treatment team colleagues, community mental health teams, early interventions teams, GPs, police, paramedics, AMHPs, and social services).

  • Clinical supervision of junior medical staff/medical students, and new non-medical prescribers,

  • Teaching and assessment of undergraduate medical students
