Band 5 Mental Health Nurse (Supported Discharge)

Job summary

We are looking to recruit an enthusiastic, motivated and experienced Band 5 Mental Health Practitioner to join our well-established Liaison Psychiatry team. The post will sit in the Supported Discharge Team within Liaison Psychiatry and the candidate will work with predominantly older adults admitted to general hospital wards aswell as in the community to provide short-term intervention.

The nurse will work across in-patient areas of Sheffield Teaching Hospitals to provide routine Supported Discharge assessments aswell as supporting safe, person-centred risk assessments to feed into discharge planning. The wider Liaison Psychiatry team carry out assessments and risk assessments relating to complex and high risk referrals.

Main duties of the job

The role includes working with service users who may have dementia, delirium, depression and/or anxiety. Following discharge from hospital, the nurse (alongside Support Worker staff) will provide short-term mental health follow-up in individuals' own homes to provide monitoring of mental health and to promote emotional wellbeing and recovery, whilst helping to reduce the risk of re-admission. The aim of Supported Discharge is to also reduce the risk of further secondary community mental health input.

The Liaison Psychiatry team provides a 24-hour service to the Sheffield Teaching Hospitals. We are looking to recruit an experienced nurse to work with older adults, during core hours of Monday-Friday, between 8am-6pm.

You will work closely with multidisciplinary colleagues across hospital and community services, supporting service users and carers through assessment and intervention aswell as signposting to further appropriate support. This role offers an excellent opportunity to develop skills in both hospital-based and community mental health care for older adults.
