Physical Health Support Worker

Job summary

An exciting opportunity has become available for a Physical Health Support Worker position at The Northlands Health Centre - North Community Mental Health Team.

The post holder will work with people under the care of the Sheffield Community Mental Health Services. They will provide support and contact with individuals in order to promote recovery from ill health and encourage the development of independence. They will be part of a multidisciplinary team from which they will receive support and supervision from the Clinical Team Manager and Advanced Clinical Practitioner. The post holder will work out in the community with a caseload of individuals and under the directions of shared care plan.

The role will involve understanding a making connection with local community and voluntary services.

Main duties of the job

The role will include acting as a Named Worker for a designated caseload to ensure service users receive a consistent approach and a named person to contact. This will involve building a working relationship and to promote an individual's ability to manage their own needs and independence. As a named worker you will work in collaboration with the Multidisciplinary team to support the overall care and treatment plan of the individual.

Job description

Job responsibilities

  • Take routine bloods, ECG, Complete Clozapine Assay, physical health monitoring i.e. BP, pulse, weight height. However, the post holder is not expected to interpret results of bloods tests, ECGs
  • Offer health promotion, dietary advice, healthy eating, smoking cessation advice, substance misuse referring to appropriate agencies in agreement with the service user.
  • Ensure appointments are re-booked within NICE guidelines and in a timely manner according to clinical need.
  • Communicate outcome of physical health screening to GPs & PCNs highlighting any physical health concerns
  • Maintain effective management of clinical areas including, co-ordinate ordering to ensure adequate stocks, safely dispose of sharp bins, equipment check i.e. defibrillator, and calibration of specific physical health equipment. Signpost to appropriate alterative services such as PCMHT and VCSE pathways.
  • Develop a rapport with service users within appropriate boundaries.
  • To encourage service users to be fully involved and engaged with their agreed care plan and to access appropriate services.
  • Implement agreed programmes of care collaboratively with service users and carers under the supervision of a named professional.
  • Help to identify early signs of relapse by monitoring the service user's progress, level of functioning and mental state and alert the appropriate staff involved in their care.
