Salary: Band 3 £24625 to £25674 pa pro rata

Hours: 1 x 37.5 hours permanent post

We are looking for a motivated and enthusiastic person to join the Older Adult Home Treatment Team (OA HTT). The OA HTT operates 7 days a week throughout the year from 8am until 8pm. We provide short term support including holistic assessments andperson centred recovery treatment plans for older adults experiencing mental health crisis.

We work with a range of mental health diagnosis including functional illness such as depression, anxiety and psychosis and also those with dementia. We operate as a multidisciplinary team which includes medics, nurses, occupational therapists and psychology. Advanced Support workers will work with the MDT with the aim to prevent hospital admission for service users and support discharge from mental health hospitals.

Main duties of the job

You will be expected to work both as part of a wider multi-disciplinary team and also working alone within the community.

The job includes working alongside the other disciplines following and implementing plans of care for individual service users.

You will work into service users own homes and care homes in the city.

You will respond to and support our service uses at times of crisis.

You will support the weekly MDT and contribute to the reviews of our service users.

You will contribute to the team meetings and service development.

Job description

Job responsibilities

Implement agreed programmes of care with service users and carers under the supervision of a named professional.

Maintain appropriate clinical records regarding the service users care observations made and information provided.

Work closely with the key worker/care coordinator to assist in delivering appropriate care to a high standard.

Help to identify signs of recovery and relapse by monitoring the service users progress, level of functioning and mental state and share this with the appropriate staff involved.

Report untoward occurrences at the earliest opportunity, to a named key worker/care coordinator so that re assessment needs can be undertaken.

Arrange and attend discussions regarding service users including handovers and a weekly MDT.

Support service users in achieving their goals by providing domestic and social support and assisting both personal health and social care including shopping, cooking, daytime activities and treatment tasks, and helping them to use mainstream facilities and activities.

Build therapeutic relationships with service users and carers and provide reassurance and information regarding the care plan.

Take a role in health promotion both with service users, carers and other staff.

Provide input into the evaluation of the service users care plan.

Encourage service users to be fully involved and engaged with their agreed care plan and access to appropriate services.

Promote equality for all individuals by promoting and supporting individuals rights and choice.

Be able to work alone in service users homes, residential care or inpatient settings using your own initiative if required.

Support and enable service users and their carers to effectively change from one care environment to another.

Observe and support service users to administer their own medication and assist with collection and transport of medication.

Take responsibility to develop and maintain skills required to achieve the competencies and deliver effective client care.
