Job summary

The South Community Mental Health Service is undergoing an exciting transformation with additional resource allocation to provide ongoing intensive support to the population of the Sheffield, this includes working towards a joint approach with Primary Care and the Voluntary Care Sector, reviewing the Community Mental Health Team (CMHT) Service Criteria and working in discreet small teams into local communities leading to a high quality care standard.

The successful candidate will be supported by their leadership team through dedicated supervision with highly experienced and motivated team managers, where learning opportunities will be supported and encouraged. From the multi-disciplinary team, we offer regular CPD sessions and candidates are encouraged to bring their own interests to the team.

You will need to possess excellent communication skills and be able to work independently and as part of the wider team in order to assess, plan and evaluate care. You should have a positive approach to meeting the needs of our service users and be motivated, empowered and enthusiastic to both learn and share.

Main duties of the job

  • Acting as a Named Worker for a designated caseload to ensure service users receive a consistent approach and a named person to contact. This will involve building a working relationship and to promote an individual's ability to manage their own needs and independence. As a named worker you will work in collaboration with the Multidisciplinary team to support the overall care and treatment plan of the individual.

  • A named worker for a Senior STR Worker would mean engaging with a service user to offer care and treatment where care is stable. If any changes occur in relation to deterioration in mental health or physical health the MDT will consider more appropriate options for that service-user in relation to the care plan/DRAM.

  • A Senior STR Worker acting as a named worker will update a care plan and DRAM and change the content with support from the MDT who would guide principles of this change

  • Contribute to the collaborative planning and review of Recovery Goals with service users and team members.

  • To have a working knowledge of care and support planning.

  • Support the completion of Patient reported outcome measures (PROMs) as part of supporting care planning and reviews of care.

  • To be aware of and abide by service policies re confidentiality and information sharing.

  • Attend and actively participate in Multi-Disciplinary Meetings and daily huddles.

  • Contribute to the provision of unplanned activity through allocated same day appointment slots.

  • Access/conduct peer/individual supervision and receive supervision from agreed supervisor on a regular, planned basis, and work within the appraisal process (PDR).

  • Contribute to the development of others through supporting students, contributing to informal supervision, seeking advice and support from others to enhance the offer of students experience.

  • To be committed to individual professional development, ensure compliance with mandatory training and support development of the STR role.

  • To maintain accurate and up to date records of all contacts and interventions with service users.

  • To undertake any other duties that maybe allocated by the Team Manager within the general scope of the post and appropriate to the grade.

Assist in the design and implementation of service development

Providing a screening service for clients under the care of the Community Mental Health team as outlined by the named worker above, ensuring the service user can access the correct information/intervention in a timely manner.

To empower and involve service users in the design of their own care and in the delivery of this and the wider service provision.

To offer a flexible service, which supports service users in times of distress or mental health relapse, involving relevant others where necessary, and increasing support on a reviewable basis.

To identify and effectively manage and escalate risk issues and taking the appropriate action in times of crisis and emergency, to ensure the safety of the service user and all concerned.

Support and provide direction to other recovery workers.

To undertake risk assessment for any work undertaken where appropriate, with particular reference to lone working policies and environmental risk.

Develop a rapport with service users within appropriate boundaries.

To encourage service users to be fully involved and engaged with their agreed care plan and to access appropriate services.

  • Implement agreed programmes of care collaboratively with service users and carers under the supervision of a named professional.

  • Help to identify early signs of relapse by monitoring the service user's progress, level of functioning and mental state and alert the appropriate staff involved in their care.
