Job summary

The South Community Mental Health Service is undergoing an exciting transformation with additional resource allocation to provide ongoing intensive support to the population of the Sheffield, this includes working towards a joint approach with Primary Care and the Voluntary Care Sector, reviewing the Community Mental Health Team (CMHT) Service Criteria and working in discreet small teams into local communities leading to a high quality care standard.

The successful candidate will be supported with their leadership team through dedicated supervision with highly experienced and motivated team managers, where learning opportunities will be supported and encouraged. From the multi-disciplinary team, we offer regular CPD sessions and candidates are encouraged to bring their own interests to the team to share and develop their specific interests in mental health.

Main duties of the job

The post holder will work with people providing support to service-users working in a discreet Care Group alongside Primary Care Networks in the South of Sheffield. You will promote positive outcomes by working with people in a proactive, practical sense encouraging service-users independence. You will work with your care team to ensure the service promotes healthy lifestyles, opportunities to enhance knowledge about mental health and provide encouragement to participate in care. You will liaise with other services to ensure a service-users journey remains as seamless as possible and report back to the MDT in a weekly meeting.

This will involve building a working relationship and to promote an individuals ability to manage their own needs and independence.
