Care Coordinator Early intervention in Psychosis

Job summary

The post holder will work within a multi disciplinary CMHT and core responsibilities will include:

o Acting as a Care Co-ordinator/Lead Professional.

o Working within Care Programme Approach.

o Comprehensive assessment of new referrals.

o Carrying a varied caseload defined by need.

o Contributing to a duty system on a rota basis.

o Delivering supervision to more junior members of staff.

o Contribute to service development.

Main duties of the job

o Triage and determine urgency of new referrals to the service.

o Carry out comprehensive assessments of an individual's health and social care needs in a variety of settings and agree the most appropriate course of action.

o To assess, evaluate and manage clinical risk.

o To formulate a plan of care based on identified need.

o Deliver evidence based interventions as appropriate.

o Complete physical health reviews.

o Review effectiveness of care plan at regular intervals.

o To work collaboratively with service users, carers and partner agencies.

o To maintain computer held care records in an accurate and timely fashion.

o Undertake all work in a culturally sensitive manner.

o Act in accordance with the Care Programme Approach, Care Management System, Acute and Schedule Care Pathways as appropriate.

o Develop effective working relationships with key stakeholders.
