Community Mental Health Nurse

Job summary

You will work closely with services in Islington, providing support for patients with Psychosis. South Psychosis Core is a well-established multidisciplinary team providing rapid, community based intervention for working age people who are in need of mental health support. The team offers specialist assessment and intervention who may be experiencing severe mental health difficulties, related to psychosis. The post holder will be part of the team, we deliver intensive support to people at risk in the community, prevent and reduce admissions, where indicated do in-reach to people to facilitate prompt and safe discharge to the community, assessment of people presenting in mental health difficulty in community settings.

Main duties of the job

To act as an autonomous, registered practitioner who is legally and professionally accountable for own unsupervised actions guided by the professional code of conduct and Trust guidelines and protocols

The post holder will have Current Effective Status on the Nursing and Midwifery Council (NMC) - Registered Nurse - Mental Health

To be responsible, and accountable, for service delivery to the patients and their families and carers.

To hold a caseload of clinical cases within the team with assessing, planning, implementing and reviewing care.

To be able to assess and develop plans of care to meet the complex needs of patients with a variety of conditions. To recognize own limitations and seek advice when necessary. This will include continuously evaluating and acting on outcome

To be able to initiate referrals to other health professional specialist services and agencies.
