Job summary
This post is for 0.6 WTE Consultant Psychiatrist working in Intensive Outreach pathways within the Core Community Mental Health of the Northern Boroughs of the trust (Enfield) as work progresses with the transformation of community mental health services in line with the NHS Long Term plan. The post is a 1 year fixed term contract, and these are new posts created from the new investment from the ICB to develop this pathway.
The Consultant Psychiatrist works closely with GPs, the Core Team MDT, and a number of intersecting systems, such as housing, social services and the Trust Intensive Teams. The focus is on relationship-building, encouraging reflective practice and collaborative working.
Main duties of the job
General Expectations of a Consultant Psychiatrist employed with North London NHS Foundation Trust include
o To work within the MDT promoting respect for all professional groups and focusing upon coherent and integrated teamwork.
o To be Section 12(2) approved and have approved clinician status and participate in the Trust-wide Section 12 daytime rota.
o To provide second opinions for Consultant colleagues if requested.
o To keep up with administration and note-keeping on the electronic patient record system in a timely manner, including letters to GPs, Dialog+ and risk assessments.
o To assist with Serious Incident investigations and complaints if requested.
o To keep up-to-date with mandatory training.
o To attend and participate in Consultants' and Management meetings to keep informed of service issues and developments.
o Other management/clinical supporting activity (to be negotiated as required by the Trust.)

The specific clinical and managerial responsibilities of the post-holder will be as follows:
To provide consultant input and clinical leadership to the local Intensive Outreach pathway within the Core Community Mental Health Teams.
To assist in the prioritization of new referrals to the pathway, which will be discussed in the weekly clinical MDT meeting of the relevant Core Team.
To proactively conduct consultant-to-consultant handover for service users being stepped up to or stepped down from the Intensive Outreach pathway.
To assist with medication reviews, Dialog+ care planning, management plans, risk assessments, safety plans (Formerly crisis plans), and self-help plans as required for these patients stepped up to the pathway.
As necessary to provide direct clinical assessments of patients and to provide follow up as appropriate.
To maintain effective communication with Intensive outreach pathway staff, wider Core Team MDT, GPs, primary care staff, patients and their carers.
To support the pathway aim of reduction in use of unplanned care services, lasting and consenting engagement and contacts with services, recovery from SMI, significant improvement in symptom severity and insight.
To provide clinical input, where necessary, to multi-disciplinary case conferencing around difficult patients being managed within the Core Teams.
To work flexibly within timetabled working hours, so that urgent clinical matters can be prioritized.
To carry out Mental Health Act and domiciliary assessments as required.
To play a key role in implementing improved interface between different pathways within the core teams.
To liaise and work closely with local voluntary services as part of the new model of core community teams
To engage with local service user participation models.
To participate in management activities and governance processes related to the Core Teams, including audits and quality improvement projects.
To play an integral role in the performance of the team, the further development of the service, and the development of clinical pathways and priorities of the service line.

Education/Qualifications/Registration
Essential
- Full registration with the General Medical Council (GMC) and a license to practice. Must be on the specialist register or within 6 months of gaining a Certificate of Completion of Training
- Membership of Royal College of Psychiatrists or equivalent higher qualification in psychiatry
- Section 12(2) and Approved Clinician status (or eligibility) - qualifies to act as a Responsible Clinician
- Registered with Royal College of Psychiatrists CPD programme and in good standing
Desirable
- Other relevant post-graduate qualification
- Trained as an Educational Supervisor for doctors in training
- Training as a Consultant mentor
Knowledge & Experience
Essential
- Knowledge and observance of the requirements and duties of a doctor laid down by the GMC in Good Medical Practice.
- Clinical knowledge of the full range mental disorders.
- Experience in the assessment of complex clinical cases involving severe and enduring mental illness, specifically those with complex mood anxiety & trauma presentations/disorders and significant risk histories and the ability to assertively engage and manage such cases.
- Expertise in a comprehensive range of evidence based interventions including psychological therapies and pharmacological treatments
- Knowledge of the Mental Health Act, Mental Capacity Act and safeguarding practices
- Experience of community psychiatry and interest and experience of working with individuals with severe mood anxiety & trauma presentations/disorders.
- Experience in work place based assessments of doctors in training
- Knowledge of clinical commissioning and external organisations that interface with an NHS Trust.
- Experience in organising and chairing meetings.
- Good understanding of Trust governance structures, especially clinical governance
