Job summary

To assess, plan, implement and evaluate client care supported through a robust preceptorship or personal and professional development programme linked to the Trust's implementation of the Knowledge and Skills

To co-ordinate the individual's assessment and subsequent care plan as agreed with the multi-disciplinary team.

The post holder will adhere to Trust policies, Data Protection and Health and Safety.

Main duties of the job

To participate as an effective member of the MDT and be responsible for the development and facilitation of a patient centred approach to care: assessing, developing, implementing and evaluating the care needs of patients and their carers (supported by a preceptor as appropriate)

To observe the requirements of the Mental Health Act 1983 ensuring the patient is fully informed of their legal position and their rights under the Act.

To determine nursing priorities and plan patient care accordingly including full risk assessment, inform the Nurse in Charge when changes in circumstances may affect standards of care.

To meet the physical, psychological, social and cultural needs of the individual and their family, using research based knowledge and skills to maintain the individual's identity.

Assist in the education of patients to having a greater understanding of their mental health condition working towards developing their strengths and coping strategies.

To administer medication in accordance with the NMC Code of Conduct, and locally agreed policies. Have a working knowledge of medication used in the treatment of mental health and be able to provide basic information regarding medication as requested by patients.

To represent the patients views in ward rounds, handover, tribunals etc under supervision.
