The Band 6 Mental Health Screener plays a vital role in the Community Mental Health Team (CMHT), conducting initial assessments, evaluating mental health needs, assessing risk, and supporting care planning. The post holder also contributes to team development, clinical supervision, and workforce planning, ensuring timely access to care and maintaining high clinical standards.

Main duties of the job

  1. Conduct initial mental health assessments using appropriate tools and frameworks.
  2. Formulate clinical impressions and care needs based on assessment findings.
  3. Assess and manage clinical risk, including safeguarding concerns.
  4. Develop and review Dialog plus in collaboration with service users and carers.
  5. Deliver therapeutic interventions appropriate to the service user's needs.
  6. Liaise with GPs, social care, and other agencies to coordinate care.
  7. Monitor and support service users' physical health needs.
  8. Maintain accurate and timely clinical documentation.
  9. Apply relevant legislation including the Mental Health Act and Mental Capacity Act.
  10. Promote recovery-focused and person-centred care.

  • Registered Mental Health Nurse with Current registration with NMC
  • Evidence of ongoing professional development and relevant post-registration training
  • Demonstrate a clear understanding of the Mental Health legislation or social care legislation relevant, its application and use in practice.
  • Experience/ Knowledge
  • Ability to communicate effectively within a multidisciplinary team. Skilled in verbal, non-verbal, and written communication, especially when dealing with sensitive or complex information.
  • High-level clinical assessment skills to screen and triage patients effectively.
  • Ability to interpret complex health and social needs, including test results and patient histories.
  • Must have Experience in coordinating care, managing caseloads, and making informed decisions based on clinical judgement
  • Full driving license and access to own vehicle
  • Proficient in using electronic patient records, digital communication tools, and data entry systems.

Desirable

  • Ability to work within teams and manage own workload
  • Experience in supporting junior staff, students, or new team members.
  • Demonstrates the ability to lead on service improvements or contribute to team development.
