Job summary

The post-holder will work as a member of the NLFT Homeless Health Service in partnership between NLFT and all relevant stakeholders.

The post is part of an initiative to improve the quality of care available to rough sleeping population in NCL boroughs through:

o Providing a responsive service that will identify mental and physical health problems.

o Working in collaboration with other services to deliver care and interventions, based on clinical need, at the right time.

o Providing interventions to support people to move off the streets and to facilitate them to sustain a more settled lifestyle.

o Referring service users on to the most appropriate service and to support them until accepted.

o Ensuring that all service users are registered with a GP.

Main duties of the job

  • To work within MDT as a member of the NLFT Homeless Health Service.
  • To work carrying out health care responsibilities with clients in non traditional settings, including on the streets.
  • To be professionally curious
  • To liaise with other agencies in planning and assessing care needs.
  • Provide formal, on-going clinical supervision and caseload management supervision to junior and less experienced community mental health staff, as directed.
  • To undertake duties designed to extend clinical and managerial expertise as delegated by the manager when required. This will include deputising in the absence of the Senior Practitioner.
  • To Undertakes risk assessment and assessment of needs level expert knowledge and clinical skills to deliver holistic care to people who are street homeless in Barnet Enfield and Haringey, who may find it difficult to access mainstream services.
  • To works in partnership with General Practitioners, Consultants, Nursing and Social Care colleagues, outreach teams, Housing related support services to treat service users with a range of acute, non-acute and chronic Mental health conditions and complex needs.
  • Refer service users as appropriate to other members of the multidisciplinary team, secondary care and to other statutory and voluntary organisations as and when needed.
  • Provides and promotes evidence based care in line with appropriate clinical guidelines and works within clinical protocols, professional guidelines and codes of conduct.
