Job summary
This is an exciting role within the highly successful Camden Memory Service Team. We are looking for an Assistant Practitioner to deliver post-diagnostic dementia support in the community.
We require a motivated individual, ideally with experience of working with older people with dementia. The post holder will need to have good communication and interpersonal skills, be able to work as part of a dynamic team, use their own initiative and build therapeutic working relations with service users and their carers.
Main duties of the job
As an Assistant Practitioner you will be responsible for offering support, advice and signposting to people affected by dementia. You will hold your own caseload and undertake reviews of people living with dementia, and their carers, living in their own homes. You will be supported by a multidisciplinary team and work closely with the lead nurse for your catchment area.
As our model aims to support all residents living with dementia, we are a busy service and caseloads are reflective of this. This requires you to have good organisational skills and an ability to work independently. The role involves visiting service users at home and you therefore must be able to get around Camden using Public Transport and/or walking/cycling.
You'll be able to demonstrate experience in working in dementia care and excellent communication and interpersonal skills. You should have some understanding of the wider health and social care system. Being computer literate is a must and you have to be able to use Word, Excel and other relevant IT systems.

The position offers opportunities to get involved in Quality Improvement projects and to become champion/lead in specific area relevant to the role.

In order to meet the needs of the new Partnership services you may be required from time to time to work at different locations to your normal place of work. This may mean that you are required to work at any location that fall under Barnet, Enfield and Harringay Trust and Camden and Islington Trust.
