Job summary

An attractive opportunity has arisen to work as a specialty doctor in the Hospital Division of Camden and Islington NHS Foundation Trust.

The post is that of a specialty doctor, spit between two in-patient wards. One of the wards is Rosewood Ward and the other is Jade Ward. Rosewood Ward, at St Pancras Hospital, is a 12 bed female treatment ward (Consultant Dr Christopher Naylor); Jade Ward, at the Highgate Mental Health Centre is a 17 bed male treatment ward (Consultant Dr Neil Sarkar). These wards specialise in the treatment of patients with severe mental illnesses such as schizophrenia and bipolar disorder. Both wards have well established, skilled and friendly MDTs. Your role as specialty doctor in these wards would be to be a senior member of the ward clinical team. You would be involved in clinical assessments and decision making.

The post includes protected time for Continuous Professional Development. There is a very active Quality Improvement department in the Trust and you would be encouraged to take part in Quality Improvement.

The post is initially for 12 months but could be extended subject to satisfactory performance.

Main duties of the job

The post is in the Hospital Division of the Trust. It is based at both the Highgate Mental Health Centre, Dartmouth Park Road, London, N19 5NX and St Pancras Hospital, 4 St Pancras Way, London, NW1 0PE. Jade Ward is at the Highate Mental Health Centre; Ruby Ward is St Pancras Hospital. However, it will be moving to the Highgate Mental Health Centre in/around December 2023, at which time it will become a 17-bed female Acute Psychiatric Treatment Ward.

Rosewood Ward:

Rosewood Ward is a female treatment ward. It specialises in the treatment of acutely unwell patients with psychosis and mania. It specialises on the assessment and treatment of patients who are acutely unwell. Patients often present with significant risks and have complex and nuanced mental health difficulties as well as physical co-morbidities. The Rosewood team comprises psychiatrists, a psychologist, nursing staff, an OT, an activity worker and support staff.

Jade Ward

Jade Ward is a 17-bed male treatment ward which has the same remit as Rosewood Ward apart from the fact that it is a male ward.

Job description

Job responsibilities

Providing a good standard of practice and care

Undertake clinical assessments of patients with mental health problems

In the in-patient wards, you will ensure that all new patients admitted under your teams care are fully assessed, including an assessment of their physical state, so that a preliminary formulation of their problems is made. All in-patients should be reassessed as necessary. This is mainly the responsibility of the CT but needs to be managed by the team. The specialty doctor must have the the capability to run ward rounds in the consultants absence (with support from a covering consultant). The specialty doctor must also be able to compose reports for Mental Health Review Tribunals/Managers Hearings and be able to attend hearings to give evidence. Some on the job training in these areas would be available before the post holder would be expected to assume full responsibility in these areas.

Manage chronic illness

Many of the patients have acute exacerbations of complex chronic mental health problems. This may include treatment resistant depression and schizophrenia, fragile bipolar disorders, chronic anxiety disorders and co-morbid conditions including personality disorders and substance misuse. These patients often require a thorough assessment, review of their diagnosis and commencing more complex treatment plans. (ILO7)

Decisions about access to care

Use the results of the clinical assessment to ensure effective patient management

Under the supervision of a clinical supervisor, you will prescribe pharmacological and psychosocial treatments for the patients that you are working with. You will refer them promptly to other professionals as directed by your clinical supervisors. You will also arrange for the core trainee to follow-up abnormal physical signs and results of investigation including arranging for repeat investigations according to patients needs and the psychotropic medications they are prescribed (for instance antipsychotics and on-going assessment of lipids, glucose, weight and prolactin). You will see patients who require urgent psychiatric assessment including Mental Health Act assessments during daytime duties, if you are section 12 approved.
