Job summary

We are the Islington Memory & Dementia Navigator Service, a high quality assessment, treatment and support service in the London Borough of Islington provided by the Camden & Islington NHS Foundation Trust, who are passionate about supporting people affected by dementia. Our unique offer supports people affected by dementia, including carers, from diagnosis throughout the entire journey of dementia. We have one of the highest diagnostic rates in the UK and have strong links with research at University College London.

We're seeking to recruit a dementia navigator/assistant practitionerwho is enthusiastic and motivated to help us deliver excellent dementia care in the community.

Main duties of the job

As a dementia navigator/assistant practitioner, you will be responsible for offering support, advice and signposting to people affected by dementia. You will hold your own caseload and undertake reviews of people living with dementia, and their carers, living in their own homes. You will be supported by a multidisciplinary team and work closely with the lead nurse for your catchment area.

As our model aims to support all residents living with dementia, we are a busy service and caseloads are reflective of this. If you have the right values for the Camden & Islington NHS Foundation Trust you will be supported by a programme of continuous training and development. The position offers opportunities to get involved in Quality Improvement projects and to become champion/lead in specific area relevant to the role.

Only candidates who demonstrate their motivation to work in dementia care will be shortlisted.
