Job summary
We are looking for a part time Nursing Associate to work alongside our adult learning disabilities health facilitation nurses and wider multi disciplinary team in the community. This role with be to support adults with learning disabilities to get their health needs met, whether these are physical health needs, mental health needs or for their emotional wellbeing. and to reduce the health inequalities people face.
The successful applicant will be working across Bradford district, and therefore ideally needs to be able to travel easily. They will be supporting people with learning disabilities where they feel most comfortable, and so may be seeing people in their home, supported accommodation, day time locations, at the GP surgery etc.
The work is incredibly varied and as a lone worker you need to be able to prioritise, risk assess and manage your work load safely. You also need to be an excellent communicator, both with the people with learning disabilities you will be supporting, as well as their families and the multiple other agencies you will be working alongside.
An ability to be creative and make reasonable adjustments to ensure people can access health support is also needed. A good understanding of the health inequalities faced by people with learning disabilities is required and previous experience of working in a similar setting would be helpful.
Main duties of the job

You will be supporting people to access health appointments, health investigations and treatments as well as being involved in assessments and monitoring of people's health needs.
You will be working closely with adults with learning disabilities as well as their main carers including families and paid staff. You will work alongside nurses in the team, the wider MDT and other agencies involved with the person's health and support needs.
