Pathway Coordinator

Job summary

We are seeking a Pathway Coordinator to support the provision of timely discharge of patients across the West Yorkshire and Barnsley ICS. Commencing discharge planning at the point of admission in collaboration with families/carers and social care where appropriate. Undertaking/ overseeing appropriate assessments and consulting with social care staff, funding panels and future care placements.

Highlighting any difficulties with placement of patients to senior management team and providing Clinically Ready for Discharge (CRFD) but delayed reports as required. Ensuring close working relationships with all professionals across the pathway both internally and externally.

Main duties of the job

  • Ensure there are clear goals of admission with estimated date of discharge agreed at admission meeting.
  • Clearly identify if the reason for admission was due to placement breakdown or lack of suitable placement in the community
  • Ensure discharge planning begins on admission with system involvement including Care Coordinator/Social Worker and Commissioner, clearly identified in the 12-step discharge plan.
  • Attends weekly multi-disciplinary (MDT) clinical review meetings, Care Programme Approach (CPA) meetings, Care and Treatment Reviews (CTR's), Best Interest's meetings and other meetings such as CTRs and DSRs planned to support discharge on both wards.
  • Completes the ATU admission and discharge tracker - 12 step discharge plan- for each patient from admission, ensuring agreed actions and timeframes are recorded and added to MDT clinical review objectives.
  • Provides a discharge update for each patient at their weekly MDT clinical review, following up outstanding actions and liaising with system partners in preparation.
  • Any barriers to discharge to be noted at the MDT clinical review meeting and escalated to line manager and Head of Programme and subsequently reviewed on a weekly basis.
  • Ensures all relevant system partners are made aware when a patient is identified as Clinically Ready for Discharge
  • Be fully integrated into the ATU teams across the system and be flexible to team pressures. Escalating any service pressures or disagreements that will potentially delay discharge

  • Registered professional degree or diploma. Which could include: RNLD, RNMH, RGN, OT, Social worker, SLT, Physiotherapist or other relevant professional qualification including psychology/psychotherapy.
