Job summary

The award winning Proactive Care Team aims to develop and deliver a community based integrated model of healthcare for individuals with the most complex health needs. The team will work in the three Community Partnership areas of the central locality in Bradford and Keighley.

Using a Lead Provider model, BDCFT will work in collaboration with key partners including Bradford Teaching hospitals NHS Foundation Trust (BTHFT) as well as the GP Federations and Voluntary Sector partners to create proactive culturally competent and aware Multidisciplinary Team (MDT). Team members will work to identify those with highest need and deliver a blend of preventative and recovery focussed interventions in the community, to support a reduction of premature mortality and help deliver improvements for those who are frail and those close to their end of life.

The overall aim of the Proactive Care Team will be to help reduce health inequalities for those with the most complex needs and support reduction in Accident and Emergency Department (AED) attendances and unplanned admissions whilst also improving access to other existing community-based provision to help sustain good health outcomes.

Main duties of the job

As part of the proactive care team you will support the multidisciplinary team in the provision of therapy services as directed by a senior therapist.

Under the supervision of senior therapists you will support in the assessment of patients and carry out interventions to enable the patient to achieve their goals in either clinic or home settings.

The postholder will take leadership from senior clinical practitioners within a delegation framework for agreed tasks. This will include supporting with a clinical caseload within the service.

The postholder will ensure high standards are maintained in a safe environment, which promotes equality and sensitivity for all individuals.

The postholder will ensure patient safety is maintained and patient experience is positive by treating all patients, family relatives and colleagues with respect, dignity and courtesy in accordance with trust values.

The postholder will support with the provision and correct use of equipment under supervision of a senior clinician.
