An exciting opportunity has arisen for a Band 6 Community Care Coordinator to join our lively and friendly Bradford City and North Community Mental Health Team for Older People. Based in the multicultural city of Bradford, we are seeking skilled, enthusiastic, and compassionate clinicians to deliver outstanding care to our diverse community. As a Community Care Coordinator, you will support clients through challenging times, empowering them toward recovery and independence. A flexible, open-minded approach is essential, ensuring all interventions align with care pathway frameworks and national standards.
We seek caring, creative professionals who thrive under pressure, connect well with others, and are passionate about helping people live independently. You will manage a caseload, coordinate holistic care, and deliver recovery-focused interventions within a supportive multidisciplinary team.
Please note: This is a fixed term 12 month contract to provide maternity cover.
Main duties of the job
As a band 6 clinician you will be expected to act as a role model and resource for our less experienced staff. You will also have opportunity to be involved in service delivery improvements and innovations.
You will be offered regular supervision and support.

You will need to demonstrate clinically effective care management skills and interventions and will have a good awareness of current health and social care issues which affect, or influence, our community. You will have excellent time management skills and, due to the community element of this role, hold a full and valid driving licence.
