Job summary
The role of the Health Care Support Worker, in the Children's Learning Disability Team, is to work with the multi professional team and the Registered Practitioner(s) to assess, plan, implement and review interventions to support children and young people with learning disabilities. This work will be in accordance with clinical governance processes and professional standards. The post holder will utilise their skills and knowledge to undertake health promotions and health interventions with the children and young people in community settings as appropriate including their home and schools.
This position is based within the Craven District and is a community based role which covers a vast geographical area. Therefore applicants are advised that the use or access to a car or suitable means of transport to travel across the Trust footprint in line with service needs may be of use as access to some public transport in unsociable hours weekends/nights may not be possible.
