Job summary
The CYP Key Worker service has been developed as a response to the NHS England Long Term Plan (LTP) commitment that by 2023/24, eligible children, young people and inpatients with a learning disability, autism - or both - with the most complex needs will have a designated Key Worker.
The service is delivered by Barnardo's and aims to provide dedicated key workers for children and young people with a learning disability and/or autism diagnosis who are already in hospital or in crisis and at risk of admission. They work with children and young people aged 0 - 25 with a focus on 14 - 25 years.
Throughout the Pilot phase the Service challenged the system to work collectively and across organisational boundaries to ensure support wraps around children, young people and their families when they most need it.
Acknowledging the complexity of navigating and bringing about significant change and transformation within statutory organisations, this role will continue to act as an enabler, facilitating cross organisational and system learning and change, creating a whole 'System' that will simplify and remove traditional barriers to accessing the right support at the right time for all families.
Main duties of the job
In partnership with Barnardo's, develop and lead on a strategic plan to support the roll out and embedding of the CYP keyworker service across the WY system
Work collaboratively with partner agencies and key stakeholders to develop and promote effective working relationships to ensure the CYP Keyworker service is embedded into wider service provision
Work closely and collaboratively within the system multi-disciplinary teams across the service and externally, providing advice and reports/ information, through attendance at strategic meetings & forums
To work in partnership with Barnardo's to support routine data collection, metrics and outcome reporting
Analyse information and utilise data to continuously learn and improve
To support contract monitoring discussions with support from corporate services
To collate, analyse and report on data as required.
To provide regular and detailed reports to the related governance and accountability groups to include attendance and representation at national CYP Keyworker service meetings hosted by NHSE.
To arrange and facilitate group meetings as required
To work in partnership with Barnardo's to support sustainability post pilot
Identify and manage any identified risks to the Service and ensure they are documented and regularly reviewed.
