Job summary

We are looking for a highly motivated individual to coordinate both our Dynamic Support Register and our Care (Education) and Treatment Reviews within the Bradford district, specifically, children with a learning disability and/or autism.

You will be working as part of the Children and Young People's Services in Bradford, primarily working with the Transforming Care Lead and the Senior Head of Integration and Change for Learning Disabilities and Neuro Diversity. You will be working closely with the local CAMHS, Keyworker, Social Care, Youth Justice and Education services.

Main duties of the job

The post holder will hold the Care, Education and Treatment (CETR) register for Bradford District Care Foundation Trust. They will arrange and coordinate CETRs in Bradford (Children's Services), alongside the Transforming Care Coordinator.

They will coordinate Dynamic Support Register (DSR) and chair the meetings. They will provide education and training around CETRs and DSR for Children's services within BDCFT

The post holder will also provide data and information to meet national reporting requirements and in support of the Transforming Care Partnership Board via a monthly CETR report

The post holder will be either based at Scorex House, Bradford or work remotely. If remotely, the post holder will be required to work at Scorex House at times when requested.
