Job summary
We are seeking applications from Mental Health Nurse colleagues to join the BDCFT Community Mental Health Assessment Team in an Advanced Nurse Practitioner role.
Applicants must have an active and relevant Nurse Prescribing qualification and experience in prescribing and other clinical responsibilities in a Community Mental Health Team service. Applicants should display a motivated, enthusiastic, and creative approach within a multi-disciplinary team and an ability make autonomous clinical and prescribing decisions in a safe and effective manner.
The role requires a concise reviewing of clinical records alongside telephone or face-to-face client contact as needed.
The post will be 37.5 hours per week. Job share could be considered if the appropriate applicants were successful.
Main duties of the job
The BDCFT (Bradford and District Care NHS Foundation Trust) Community Mental Health Assessment Team (CMH AT) offer a multi-disciplinary, pro-active, equality based and comprehensive triage and assessment service to residents of Bradford, Airedale, Wharfedale and Craven who are registered with a GP within the BDCFT area, and who may be experiencing severe and enduring mental health issues, have complex needs, and may need specialist or other interventions to support their needs. The team is comprised of Community Mental Health Nurses, Local Authority Social Workers, Non-Medical Nurse Prescribers, and Clinical Psychology professionals. We offer an assessment /intervention period, during which we aim to develop an appropriate and individualised support pathway. This may take the form of advice re support options available in the non-statutory and voluntary agency sector, other specific advice in relation to treatment and support options available through GP and primary level services, referral to other specialist services within the BDCFT , and /or referral onto the Recovery CMHT (Community Mental Health Team) services for support and intervention if mid and longer term needs indicate this is required.
