Transition Health Facilitation Nurse (Learning Disabilities)

Job summary

We are looking for an experienced learning disabilities nurse, to help us develop a new part time role in our team. This is an exciting opportunity to work within our learning disabilities services providing support to young people with learning disabilities or those who are autistic, with a focus on 1) improving access to dental care and 2) improving access to primary care & supporting with health assessments.

The person will work 2 days a week with our clinic and dental team to support young people with learning disabilities or autistic young people aged 14 - 18 who require significant reasonable adjustments making in order to ensure they can access dental care under intravenous sedation in primary care, or dental admissions for treatment under general anaesthetic at Bradford Royal Infirmary or Airedale General Hospital; in particular, supporting them with a hospital admission plan.

They will also work 2 days a week to help ensure young people with learning disabilities aged 16-18 who are on the Children's Dynamic Support Register have additional support to access primary health care.

Main duties of the job

The post holder will work collaboratively with other professionals and agencies to provide a high quality, comprehensive and dynamic specialist nursing services with the central aim of maintaining and improving the health and wellbeing of young people with learning disabilities or those who are autistic. You will need to promote and support the young people and their families to have a smooth and coordinated Transition from children's health services to Adults health services.

This will include working directly with service users who have complex healthcare needs, supporting others who act as Health Facilitators to people with healthcare needs of lesser complexity and also engaging in a range of health promotion activities.
