Job summary
Working for the community of Bradford District offers a great opportunity to support a diverse population.The Health Visiting service in the Bradford District is involved in several projects to support the public health agenda both with our local authority partnerships and with Public Health England.
The MESCH parenting support program offers staff training to support vulnerable families across the district. The program has been piloted and now is being rolled out across the full district.
In the Bradford East locality, we work with The Better Start Bradford Program which is funded by The National Lottery Community Fund until 2025. Better Start Bradford works with expectant families and families with children aged 0-3 to help give children the best possible start in life as highlighted in the 1001 Critical Days agenda. They have developed and commissioned a range of innovative projects to address the themes of a child's social and emotional development, language and communication and health and nutrition. These projects are evaluated by the Innovation Hub, linking to Born in Bradford, and the learning disseminated across the Bradford area.

Main duties of the job
To lead and co-ordinate the delivery of the Health Visiting & School Nursing Delivery Model (May 2021) and programmes of care to an identified caseload of children within the Bradford District in order to support them to reach their full potential
To provide a professional public health service based on best evidence to families, groups and communities; enhancing health and reducing health inequalities through a proactive, "Universal in reach - personalised in response" approach for all children 0-5 years and for vulnerable populations, targeted according to need.
To plan, co-ordinate and deliver programmes of care in conjunction with families and the multi-disciplinary team and undertake continuous ongoing assessment of children, young people and families. The emphasis of skill mix and integrated working is vital to service delivery, building on existing good practice.
