Job summary

This role is an exciting opportunity for a registered band 6 mental health professional (Nurse, Social Worker, Occupational Therapist or Children's Wellbeing Practitioner) to work in a dynamic way across primary and secondary care to bring together mental health pathways and support the understanding of mental health within primary care. The role will provide holistic assessment of services users linking people into the community and VSCE resources as well as providing seamless transition into secondary care if it is needed.

The primary care mental health service is expanding and is a key component to our community mental health transformation. It is envisaged that the role will closely link to the community transformation of mental health and the core team that is being established to work with primary care networks.

The role will link to a specific primary care network and work closely with the practices aligned to that network. The successful candidate will be based in GP sites within the PCN during working hours. Role will also include remote working from home/Trust sites.

Main duties of the job

The primary role of the post holder will be to provide specialist support, advice, and training to Primary Care Networks (GPs), thus aiding recognition of child and adolescent mental health disorders at an early stage and advising on strategies for interventions. You will play a role in establishing links, educating, and communicating with Primary Care Networks and community-based services. Providing training and a consultation service to them and other professionals and agencies who work with young people and families. This postholder will also work to support the delivery of a high-quality link provision into primary and secondary care. This will include providing consultation and training to staff in a range of agencies in relation to the mental and emotional well-being of children and young people.

The SMHP will work collaboratively with young people and their families to:

a. support shared decision-making around their care and support working on strengths and assets.b. promote recovery and self-care.
