Job summary

We are looking for 2 enthusiastic and positive Community Mental Health Nurse professionals to join the evolving Community Mental Health Assessment Team . The posts are at Band 6 level and 37.5 hours per week. Other hours could be considered if the appropriate applicants were successful in a job share format.

Applicants must have an active and relevant Mental Health Nurse qualification and experience within the community mental health delivery area , as well as being motivated, enthusiastic, and creative within a multi-disciplinary team.

The BDCFT (Bradford and District Care NHS Foundation Trust) Community Mental Health Assessment Team (CMH AT) offer a multi-disciplinary, pro-active and comprehensive triage and assessment service to residents of Bradford, Airedale, Wharfedale and Craven and who may be experiencing severe and enduring mental health issues, have complex needs, and may need specialist or other interventions and support.

Main duties of the job

The team is comprised of Community Mental Health Nurses, Local Authority Social Workers, Non-Medical Nurse Prescribers, and Clinical Psychology professionals. We offer a variable assessment /intervention period, during which we aim to develop an appropriate and individualised support pathway for each person. This may take the form of advice re support options available in the non-statutory and voluntary agency sector, other specific advice in relation to treatment and support options available through GP and primary level services, referral to other specialist services within the BDCFT , and /or referral onto the Recovery CMHT if mid and longer term needs indicate this is required.

The role would be across the BDCFT catchment area.
