Job summary
The Self- Management Facilitator (SMF) is a new role to Community Nursing in the Airedale and Bradford locality and sits within the Anticipatory Care portfolio.
The role offers a new opportunity of working, providing dedicated time to support patient and carers to become self caring and independent with support, learning and education from the Self-Management facilitators. Ensuring that care is provided by the right people and, patients and carers are empowered in managing their Health Care.

Main duties of the job
The main duties of the role would be to work alongside other community services such as District Nursing and Pharmacy technicians to identify patients and carers that could be supported to be self caring. Potential areas of teaching could be around medicine administration such as Insulin and anti-coagulants, wound care, catheter and stoma care to name but a few.
Support and training will be provided to SMFs to help develop clinical skills, knowledge and competencies and, also with motivational interviewing skills to assist patient and carers with perceptions, fears and concerns.
