Job summary
We are looking to recruit a highly motivated, forward thinking, committed and enthusiastic Nursing Associate with excellent communication skills to join our Community Diabetes team. You will be pivotal in providing care to the population of Luton working across both the Diabetes team and Adult Community Nursing team, and act as a role model to current and future trainee nursing associates.
The role will focus on those patients who manage Diabetes as a long-term condition, therefore skills will be required in administering insulin and other care needs, promoting patient teaching and independence in managing their condition, alongside, delivering education to patients and professionals and ensuring their Diabetes housebound review is up to date.
You should have excellent clinical, written and verbal communication skills and have completed the Trainee Nursing Associates course, gaining NMC registration.

Main duties of the job
Provide compassionate, safe and effective care and support to people in a range of care settings.
Monitor the condition and health needs of people within their care on a continual basis in partnership with people, families, and carers.
Contribute to ongoing assessment recognising when it is necessary to refer to others for reassessment
Communicate effectively with colleagues, providing clear verbal, digital or written information and instructions when sharing information, delegating or handing over responsibility for care
Working alongside senior health professionals in planning the work load and completing tasks on Sys1/Malinko
Active involvement in the prevention of and protection against disease and ill health
Engage in public health, community development, and in the reduction of health inequalities
To liaise as appropriate with members of the Adult Nursing team.
Develop good working relationships and liaise as appropriate with parents/ carers, and other health, social or voluntary agencies.
To be actively involved in the communication of complex information regarding clients' needs and any other useful information to the Adult Nursing Team, and/or other appropriate medical services (such as GP) and members of the wider Adult Services team.
