Job responsibilities

Provide skilled paediatric nursing care to children and young people within their own home, school or nursery settings to ensure families are supported in the care of their children in the community.

Ensure continuity of care is established and appropriate support available for children, young people and families, with the aim of preventing unnecessary admission or attendance at hospital and facilitating earlier discharge where appropriate from both local and tertiary hospitals.

Be responsible for planning and co-ordinating care, delivering and evaluating the care given to children and young people. Must be able to use own initiative and work autonomously to provide skilled care.

Supervision and support available from Lead Nurse.

Promote the health, comfort, dignity, privacy and quality of life for children young Work within the limits of the NMC code and guidelines, Trust policies, service operational policy and own professional competence.

Liaise with other members of a multi-professional multi-agency team regarding clinical management of patients to ensure effective treatment, by organising, liaising and coordinating care, taking the lead in decision making when appropriate.

Ensure prescribed nursing care is carried out in line with local and national policies including administration of SC / IV / Central IV treatments, taking of blood samples or specimens as required.

When appropriate ensure prescribed medication is administered correctly and safely (single checker procedure).

Clinicians with a V300 or V100 qualification are required to undertake a prescribing function within the role, which will include; maintaining their current registration with their professional body. Ensure they register with the Trust prescribing administrator. Fulfil all relevant continuing Professional development requirements. Identify their own development needs in regard to prescribing. Work in accordance with the Trust Medicines Policy (DN014) and relevant procedures.

Competently deliver / advise on specialist evidence based practice within the community setting as required e.g. wound management / gastrostomy care / oxygen dependency / portacaths / technology dependent children.

Provide skilled, evidenced based, sensitive, comprehensive care to children and young people who require end of life care through the 24 hour period. Liaise closely with other agencies to ensure good care assessment and delivery allowing the child, young person and family choice at the end of life stage.

Main Duties and Responsibilities

To assess, plan, implement and evaluate the nursing care and support requirements for children with medical, health needs.Maintain the safety of children and young people by ensuring robust care plan are in place with the supervision of the senior team.

To liaise with children and their parents/carers along with other agencies who are involved in their care to promote a person centred outcomes approach to supporting their needs.

To support the provision of acute care to children and young people following agreement of care from the wider teams.Ensure prescribed nursing care is carried out in line with local and national policies including administration of SC/IV/CVAD IV treatments, taking of blood samples or specimens as required.

To provide theoretical training and competency-based assessments in relation to nursing interventions for staff working in partner agencies including schools (mainstream and special needs), colleges and early years settings.

To support the Leadership Team in developing service improvements to ensure the health needs of the children and young people who use the service are met.

Communication

Skilled communication with children, young people and families who use the service.

Address compliments, comments and complaints about the service in a sensitive and collaborative manner to ensure that service improvements are identifies and implemented.

Communicate effectively with key stakeholders, including Children and Young People and their families, statutory and non-statutory organisations and partner agencies.

Deliver training and project presentations to colleagues within and outside of immediate service.

People Management

Development of staff.

Supporting seniors nurse with line management of non-registrants and delegation of clinical tasks.

Maintaining own professional development and requirement to take part in appraisal and KSF process.

Research & Development Activity

Required to undertake R&D activity to support contemporary nursing practice and quality service development.

Clinical and Practice Governance

Observe and maintain strict confidentiality with regards to any patient/family/staff/records and information in line with the requirements of the Data Protection Act.

Any data that is taken/shared as part of a phone call or transported, faxed or transferred electronically must be undertaken with regard to the Trust Information Governance and Information Security policies.

It is a condition of your employment that you are currently registered with the Nursing and Midwifery Council (NMC) and it is your responsibility to maintain your professional registration.

Undertake mandatory training and any other training relevant to the role as required by Cambridgeshire Community Services NHS Trust.

The post holder must participate in clinical and Safeguarding audits as required.

Provide patients and their families / carers with information on standards they should expect from the team.

Participate in clinical supervision on a regular basis.

The post holder is required to participate in relevant emergency preparedness process for their team.

Person Specification

Qualifications & Training

Essential

• RSCN or RN Child
• NMC registration

Experience

Essential

• Evidence of working with children and young people with complex medical health needs
• Accepts responsibility and accountability for own work and can define the responsibilities of others
• Demonstrates understanding of safeguarding issues
• Ability to transfer clinical skills to a variety of settings.

Desirable

• Experience of working in a community setting
• Student Assessor training
• Palliative Care Experience

Knowledge & Skills

Essential

• Ability to work effectively as part of the inter-agency team
• Excellent verbal and non-verbal communication skills
• Ability to prioritise and make decisions
• Good organisational and time management skills

Desirable

• Experience of CVAD lines
• Certified competency in phlebotomy
• Ability to assess, plan and prioritise care for a group of patients
