Job summary
The Norfolk Teenage Parent Pathway was devised and successfully implemented in September 2017.
The pathway is structured to deliver an enhanced service to young parents in Norfolk, to achieve the best outcomes for young parents and their children. We are seeking applications from confident, enthusiastic, and committed assistant practitioners/nursery nurseries who are seeking an opportunity to further develop their skills in working with some of the most vulnerable parents and parents-to be in the County.
Norfolk Teenage Parent Programme is an adaptation of Norfolk Pathway 2 Parenting programme delivered on a 1-1 basis. The programme commences in pregnancy, and continues throughout the first 2 years of the child's life, promoting positive care-giving and with a focus on early intervention/child development to give these infants the best possible start in life.

Main duties of the job
You will have experience of working in the community with families, are service user and team focused and have a desire to work with young parents and parents-to-be. We will provide you with in-house training, regular supervision and an opportunity to gain great job satisfaction.

Job responsibilities
- Provide additional intervention (The Norfolk Teenage Parent Programme) to teenage parents in Norfolk to ensure their child(ren) achieve the best possible health outcomes.
- Be supported by the wider 0-19 team in order to ensure needs assessment of teenage parents and parents-to-be are met.
- Make assessment of the child(ren)s developmental progress and agree levels of intervention required with caseload holder to optimise developmental and social/emotional progress.
- Support the 0-19 team in providing an innovative and high quality service in an environment that enhances client care and promotes child and family wellbeing.
- Be an active member of the HCP 0-19 team delivering the Teenage Parent Programme and facilitate delivery of the Healthy Child Programme.
- Act as first contact to teenage parents/parents-to-be in outreach locations ensuring a high standard of client care.
- Liaise regularly with the wider teenage parent pathway staff members to assist in the co-ordination, development and quality-improvement of the service.
- Hold awareness of the increasing vulnerability of this particular caseload.
- Participate in regular management, clinical, safeguarding and group supervision and team training.
- Ensure safety and dignity of service users at all times.
- To promote the use of digital resources on Just One Norfolk to enhance self-care skills for children, young people, and their families.
- To confidently utilise virtual platforms in assessments, interventions, training, and networking to support effective practice and partnership working
- To support other localities including the Single Point of Access to ensure that all relevant clinical skills are kept up to date, and to maintain effective working relationships and continuity of services
- Within this role you will need to travel as an independent practitioner on a daily basis, across Norfolk which involves both rural and urban travel. The nature of our work does result in travel arrangements changing at short notice. Due to the large geography our service covers and the limitations of sufficient public transport to undertake this role you must hold a full and current driving licence and have access to your own transport. This is subject to the provisions of the Equality Act 2010, and therefore if the postholder requires a reasonable adjustment due to a disability then this will be considered.
